Avoiding Rejection: A Guide to the Illinois Birth Certificate Apostille Process

Birth Certificates: Must be certified by the County Clerk or local registrar where the birth, death or marriage occurred.
Copy Document: certified as true copies by eligible State of Illinois official(s) or eligible document(s) properly notarized by a commissioned Illinois Notary Public

Rules for Birth Certificate

  • Use the Correct Document

    You must submit a "Certified Copy" issued by the Illinois Department of Public Health, Division of Vital Records. It must have the original raised seal and signature from the State Registrar

  • No Notarization Required (Do Not Do It)

    You should NOT have your certified Illinois birth certificate notarized. The apostille is placed directly on the certified vital record itself, authenticating the signature of the State Registrar. Notarizing it is a common reason for rejection.

  • Ensure the Document is Original and Unaltered

    The certified copy must be in its original condition. Do not staple, tape, or hole-punch the document. The raised seal and signature must be completely visible and intact.

  • Complete the Request Form

    You must include a completed "Authentication/Apostille Request Form" with your submission. This form provides your contact information and return address.

  • Include Payment and Return Envelope

    Submit the correct fee (check or money order payable to "Secretary of State") and a self-addressed, stamped envelope for the return of your document.
