Rules for Death Certificate
Must Be a Certified Copy
You must submit an original certified copy issued by the Illinois Department of Public Health, Division of Vital Records. It must have the official seal and the signature of the State Registrar or County Clerk.
Certificate Must Be Recent
Illinois has a firm recency requirement. The certification date on the vital record must be within one year of the date it is submitted to the Illinois Secretary of State for the apostille.
Do Not Notarize the Certificate
The certified death certificate itself should not be notarized. The apostille is applied based on the signature of the vital records official. Notarizing it will invalidate the document for this process.
Complete the Request Package
